The Home and Community-Based Services (HCS) program provides individualized services and supports to people with intellectual and developmental disabilities who live with their families, in their own homes, or in other community settings—such as a small group home with no more than four residents.
There are three primary living options available through the HCS program:
- Host Home/Companion Care – This option allows an individual over the age of 18 to live in a private family home. The Host Home Provider may be the person’s biological parent or an experienced caregiver who provides daily care and support. In this arrangement, the Host Home Provider is employed by Aim High and receives financial compensation for meeting the individual’s daily needs.
- In-Home and Community Support (CFC PAS/HAB) -This option serves individuals who live in their family home or independently in the community. Through Community First Choice (CFC) PAS/HAB, Aim High provides direct support to help with daily living activities and skill development to promote independence and community inclusion.
- Services may include assistance with personal care needs—such as bathing, dressing, meal preparation, or housekeeping—as well as training and support to build and maintain skills like communication, budgeting, and social interaction. CFC PAS/HAB services are person-centered and flexible, tailored to each individual’s goals and support needs.
- Residential/Group Home – This option serves individuals who prefer to live in a small group home setting with three to four people of similar abilities, receiving 24-hour supervision and support while maintaining community integration.
