At Aim High Home and Community Services, Inc., our Case Management team works closely with each individual, their family, and the Local Intellectual and Developmental Disability Authority (LIDDA) to ensure that services are person-centered and truly meaningful.
Our case managers actively participate in Service Planning Team (SPT) meetings alongside the LIDDA’s Service Coordinator to develop individualized plans tailored to each person’s goals, and support needs.
Once a plan is in place, our case managers:
- Coordinate with staff and providers to implement services as outlined in the person’s plan of care.
- Monitor progress, ensure quality service delivery, and adjust supports as needs change.
- Advocate for the individual’s rights, preferences, and inclusion within their community.
- Maintain consistent communication with families, guardians, and team members to promote continuity and trust.
